Support at Home

Mutkin Community Care Support at Home Program provides services to help you stay at home as long as possible by supporting your assessed needs. There are four levels of Support at Home which are designed to give you the care you need now, but can also be changed if your needs increase later.

We give you choice and flexibility in the way care and support is provided to you and arrange your home visiting schedule that’s is suited to your needs by the Community Care Service team.

Community and Family Life

At Mutkin, we believe it is vitally important to stay engaged with family life and the community. Our passionate team are committed to providing you with a range of activities for you to get involved with every day. Whatever your passion may be, or if there’s something you’re interested in that isn’t on offer, speak with our team who are always excited to hear new ideas and suggestions.

We have an activities planner with daily activities and programs and special occasions that are celebrated at least once each month. Family and friends are invited and encouraged to participate as much as possible in our lifestyle activities. These include cognitive activities including puzzles, cultural and spiritual activities, in-home concerts, day trips, movies, karaoke, NAIDOC activities, pamper days and exercise program. Mutkin also accommodates visiting groups from churches, schools, TAFE colleges, community groups and organisations internal and external to Yarrabah.

Eligibility

Support at Home require clients to be assessed by an Aged Care Assessment Team (ACAT).

You may be eligible for a Support at Home program if you are:

  • an older person who needs coordinated services to help you to stay in your home
  • a younger person with a disability, dementia or other special care needs that are not met through other specialist services.

Accessing the service

To access this service you will need to contact My Aged Care. Visit www.myagedcare.gov.au to find information about aged care and accessing the services that are right for you or call the My Aged Care contact centre on 1800 200 422 from 8am – 8pm Monday to Friday and 10am – 2pm Saturday (your local time, no matter where in Australia you live).
